Terms and Conditions

Mobile Martial Arts (MMA), Karazenpo Go Shinjutsu (KGS), and the Karazenpo Go Shinjutsu Black Belt Society (KGSBBS) reserve the right to dismiss or deny instruction to any student at any time for any reason. By signing this application, I (The student, parent, or guardian) hereby acknowledge that the above statements are true. I also am aware that the Martial Arts are a contact sport, and that I or the student may be subject to minor contact with the instructor or other individuals, which could result in minor or major injuries. Therefore, I do not hold any of the instructors associated with Mobile Martial Arts, the Karazenpo Go Shinjutsu system, the Karazenpo Go Shinjutsu Black Belt Society, or any of its affiliates responsible for any injuries I may receive on any premises where instruction is taught (____________________________________________________________________), and I acknowledge that all injuries incurred to myself at this premises are my fault (______). I understand that my monthly payment is due on the day of the month negotiated with the chief instructor, founder/owner of the business, or any of its employees. I understand that it is my duty to know my due date in regards to monthly payments (______). Monthly tuition is non-refundable and is paid in advance (the last class day of each month) for the following month. A reduction in tuition will only be given if a specific amount of vacation time was requested by the student and approved by the chief instructor or owner ahead of time, or unless certain terms or conditions were agreed upon in a written contract signed by both the chief instructor, founder/owner of the business, or any of its employees. By signing this application, I attest that I have read and understand all the rules and regulations associated with Mobile Martial Arts, and that all my questions have been answered in full detail, and that I have no further questions.
